Ders Değerlendirme Anketi, çoktan seçmeli maddelerden oluşan dört bölüm ve açık uçlu sorulardan oluşan bir bölüm olmak üzere beş ana bölümden oluşur.

Toplam 16 maddeden oluşan çoktan seçmeli bölümlerde, ilgili ders, ‘Ders Tasarımı, Öğretimin Etkililiği, İletişim ve Etkileşim, Değerlendirme ve Geri Bildirim’ boyutları açısından 1-5 skalasında (5: Excellent, 1: Poor) değerlendirilir.

2 sorudan oluşan açık uçlu bölümlerde ise ilgili dersin güçlü yönlerinin yanı sıra gelişime açık yönleri hakkında görüş bildirilmesine olanak tanınır.
